Info for New Members
Whether you are an incoming 7th grader or already established in high school, this is a great starting point for our new members and parents to embark on an amazing experience!
Absence Request Forms
If your child is going to miss a practice or performance, make sure to complete the necessary forms at least 48 hours in advance.

Medical Forms
Please note that if your child requires medication during any band event (even over the counter medication) you MUST fill have your doctor fill out the forms below.
The nurse typically carries the following medications: Tylenol (325mg, 500mg), Ibuprofen, Tums, Pepto, Meclizine or Dramamine, Benadryl, Lactaid and basic first aid supplies.
